Booking Your Trips You may make arrangements to book a trip by calling the Sunshine Coach Service Office between the hours of 7:30 a.m. and 4:30 p.m. Monday to Friday at 613-432-2134. Please give us as much notice as possible to ensure that we will have a vehicle and driver free to accommodate your request. Application For Transportation If you have not previously been a client of the Sunshine Coach and would like to begin using our service or would like to arrange service for a parent or child, please follow the instructions in this section. Since Sunshine Coach Service is dedicated to serving the transportation needs of people whose mobility impairment renders the use of conventional modes of public transportation inappropriate to their needs, we require completion of an Application For Transportation. This form consists of two main sections. The first section is the Application Information area inwhich you provide information about yourself which will be valuable to us once you have become a client. The second portion of the form is the Doctor's Section which must be completed by your family physician. The duly completed form may be forwarded to us by land main, by fax or by e-mail where it will be retained in our office files in total confidence. We provide a version of the form in Adobe PDF format, via the link appearing below. You may open and print the blank form which is not intended for completion online. Alternatively, you may pick up a copy of the form from our offices located at 44 Railway Avenue in Renfrew during regular business hours, Monday to Friday 7:30 a.m. to 4:30 p.m. Application For Transportation - Adobe PDF Version Payment Options Sunshine Coach offers several ways in which clients my pay the fare for their trips: Cash Fare - most trips are paid on a cash basis at the time trip is completed. Fare Passes - are available from any of our drivers or by contacting our offices during regular business hours. Each fare pass costs $90.00 and provides a convenient method of paying for trips without the need for carrying cash. The driver will validate your trip by punching your pass. Client Account - clients who are frequent users of the service may apply to our Operations Department to create an account against which their trips may be charged. An invoice covering the costs of trips taken within the calendar month is provided at each month end.
Online Bookings - Booking Agents Sunshine Coach Service provides online booking capability for Health Care Provider partners in Renfrew and area through accredited Booking Agents.
